NDIS Audit Preparation: How to Pass Your Audit First Time
Every NDIS provider — whether registering for the first time, renewing their registration, or undergoing a mid-term review — must complete an audit against the NDIS Practice Standards. The audit is the single biggest hurdle in the registration process, and the single biggest reason applications fail.
The difference between providers who pass on the first attempt and those who receive non-conformities almost always comes down to preparation. Not just having the right policies on paper, but demonstrating that those policies are genuinely understood and implemented across the organisation.
Sky Staff’s audit preparation service closes the gap between what your organisation has documented and what an auditor will expect to see. We conduct a full mock audit, identify every gap, and resolve them before your real audit takes place.
Types of NDIS Audits
Verification Audit
A verification audit is a desktop review for providers delivering lower-risk, lower-complexity supports. The auditor reviews your documentary evidence against the relevant NDIS Practice Standards without visiting your premises. Verification audits typically assess four quality indicator areas and can often be completed in half a day. Cost: approximately $900–$1,800.
Certification Audit
A certification audit is a comprehensive, two-stage assessment for providers delivering higher-risk or more complex supports (including SIL, SDA, behaviour support, HIDPA, and early childhood). Stage 1 involves a documentation review (usually off-site). Stage 2 involves an on-site visit with staff interviews, participant interviews, and direct observation of service delivery. Cost: approximately $3,000–$12,000+ depending on scope and size.
Mid-Term Audit
Certified providers must complete a mid-term audit 18 months into their 3-year registration period. This is an on-site review to verify ongoing compliance. It is less comprehensive than the initial certification audit but still requires up-to-date documentation and evidence of continuous improvement.
Renewal Audit
Every provider must renew their registration every 3 years with a fresh audit. Renewal audits follow the same format as initial registration audits (verification or certification depending on your registration groups). For information on renewal costs, see our registration costs guide.
What Auditors Actually Look For
Understanding the auditor’s mindset is critical to passing your audit. Auditors are not looking for perfect organisations — they are looking for organisations that have genuine systems in place and can demonstrate that those systems work. The NDIS Commission’s audit guidance sets out what auditors assess.
For Verification Audits
- Documentary evidence that your policies meet the relevant Practice Standards
- Proof of current worker screening checks and qualifications
- Evidence of a functional complaints and incident management system
- A continuous improvement register showing documented actions (even if you’re a new provider with no incidents yet, the system must exist)
For Certification Audits (the above, plus)
- Key Personnel who can explain your policies and governance in their own words — not recite them verbatim. This is the most common failure point.
- Staff who understand person-centred practice and can describe how they support participant choice and control
- Participant evidence: participants (or their representatives) confirming that the service they receive matches what’s documented
- Physical environment compliance: safe, accessible, appropriate to the support being delivered
- Evidence of active risk management — not just a policy, but documented risk assessments for real scenarios
- Training records showing ongoing professional development, not just initial induction
The 7 Most Common Reasons NDIS Audits Fail
Based on our experience preparing providers across Australia, these are the most frequent causes of audit non-conformities:
- Generic policies. Template-based documentation that doesn’t reflect the provider’s actual operations. Auditors spot these immediately because the language is generic, the procedures don’t match the service model, and staff can’t explain them. The NDIS Commission has explicitly warned against this.
- Key Personnel can’t explain the Practice Standards. During certification audits, auditors interview your leadership team. If they can’t articulate how governance, risk management, and quality systems work in practice, it raises serious concerns about organisational capability.
- Missing or expired worker screening. Every person in a risk-assessed role must have a current NDIS Worker Screening Check. Expired checks or missing documentation is an immediate non-conformity.
- No complaints or incident register. Even if you’re a new provider with zero incidents, you must have a functioning system ready to capture, manage, and report incidents within 24 hours. The register itself is the evidence.
- No continuous improvement evidence. The Practice Standards require evidence that your organisation actively identifies areas for improvement and acts on them. An empty continuous improvement register suggests the system exists only on paper.
- Insurance mismatches. Your insurance certificates must exactly match the legal entity applying for registration. Different entity names between your ABN, insurance, and application is a common and easily avoidable error.
- Inadequate risk assessments. Having a risk management policy is necessary but not sufficient. Auditors want to see specific, documented risk assessments for the services you deliver and the environments you operate in.
Sky Staff’s Audit Preparation Service
Our audit preparation service is designed for providers who want to maximise their chances of passing first time:
- Comprehensive gap analysis: We review every policy, procedure, and piece of evidence against the applicable Practice Standards modules and identify exactly what’s missing, incomplete, or non-compliant.
- Mock audit: We conduct a full simulation of your audit — including Key Personnel interviews for certification providers — using the same framework JASANZ-accredited auditors use. You’ll know exactly what to expect on audit day.
- Remediation: We fix every gap identified in the mock audit. This may include rewriting policies, creating missing registers, developing risk assessments, or building out your training records.
- Key Personnel coaching: We prepare your leadership team for auditor interviews with scenario-based practice sessions. We ensure they can articulate the Practice Standards in their own words, using real examples from your operations.
- Evidence pack final review: Before your audit, we do a final check of your complete evidence portfolio to ensure nothing has been missed.
- Audit-day support: We’re available on the day of your audit for questions, clarifications, and to help resolve any issues that arise.
Once you’ve passed your audit and received your registration, Sky Staff can provide trained offshore Virtual Assistants from $8/hr to manage your ongoing compliance administration — worker screening tracking, incident reporting, continuous improvement documentation, and more.
